Foundation Health Plan, a Sacramento-based nonprofit HMO founded by physicians in 1978, created the HMO Foundation Health Plan in 1983, which in 1984 establishes Sierra Foundation for Health to “fund and support the health and health-related needs and concerns of the public.”
